Eight plan designs available, including HMO, PPO, and two Covenant-only plans

Eligibility Requirements

To be eligible for the chamber plan, an employer must

Be a member of the Lubbock Chamber of Commerce
Operate in FirstCare's service area
Have a minimum of two verifiable employees
Cover at least 75% of employee-only premium
Have 75% of eligible employees participating
